Data Days
How many days of data are required?
CMS states the rule as a range tied to the code descriptor: data must be collected for 2 to 15, or 16 or more, days out of 30 days, depending on which code is being reported.
Most vendor guidance still states a flat requirement of 16 days out of 30. That was the rule for the single device supply code, and it is out of date now that the code set distinguishes a shorter collection period.
The day requirement does not apply to the treatment management codes. CMS names 99457 and 99458, along with the remote therapeutic monitoring management codes 98980 and 98981, as outside it. Those codes are governed by documented time, not by transmission days.
This page deliberately does not assign a specific day count to each individual code. The code set was revised for CY2026 and the authoritative per-code table is the current CMS specification. A precise rule is more useful than a stale number.

What can run alongside it?
Either remote patient monitoring or remote therapeutic monitoring may run concurrently with chronic care management or transitional care management, but not both.
Remote therapeutic monitoring is a parallel code family, covering non physiologic data such as therapy adherence and response. It is reported on its own codes rather than on the remote patient monitoring codes, and it was also revised for CY2026.
As with every care management program, time counted toward remote patient monitoring treatment management cannot also be counted toward another billed service in the same month.

Can remote patient monitoring and remote therapeutic monitoring both run with chronic care management?
No. One or the other may run concurrently, not both. They are alternatives rather than complements for a given patient.
